Kathleen Brophy, RN,
Nursing Supervisor

Kathleen Brophy

Kathleen, a registered nurse whose career spans over 35 years in the healthcare industry, has extensive experience in geriatric, obstetrical and pediatric nursing. She is responsible for the daily operations of the home care office and supervises the certified home health aides who care for clients in both facilities and private homes. She enjoys working with Springpoint at Home clients and their families and finds satisfaction in knowing that the services we provide help people live a safer and more enjoyable life. Kathleen is a graduate of Helen Fuld School of Nursing and Mercer County Community College.


Lisa Candito,
Facilitator

Lisa Candito

Lisa has over 23 years of experience in the healthcare industry. Prior to being promoted to facilitator, she managed the scheduling for Meadow Lakes, Monroe Village, and The Atrium at Navesink Harbor. She is responsible for the overall operations of Springpoint at Home, including accounts payable, accounts receivable, resident billing, payroll, and other business-related functions as well as general accounting for Springpoint Senior Living corporate office. Lisa also oversees telephone requirements as needed for long-term care insurance and provides support to intake screening coordinators, supervisors, and the director. Her medical training and certifications include corporate compliance training, certification as a Specialized Clinical Technician and Medical Assistant.


Rachel Glatt, MSW, LSW, CALA ,
Geriatric Care Manager

Rachel Glatt

Rachel has been a care manager for Springpoint at Home since 2016. Prior to this, she worked at Greenwood House, a provider of wellness-focused senior lifestyles and healthcare services. Her expertise as a care manager is in placement and facility advocacy. She is a licensed social worker, a certified assisted living administrator, and a member of the National Association of Social Workers. Rachel holds a Master of Social Work with a certification in Gerontology from Temple University.


Barbara Goodman, MSW, RN, LSW,
Geriatric Care Manager

Barbara Goodman

Barbara has extensive experience working with older adults. Her specialty as a care manager is in nursing/medical advocacy and community resources. She has served as a social worker in a long-term care community and an affordable housing apartment building, a senior services social worker in a family service agency, and a senior move manager. She is a New Jersey licensed social worker and has an inactive state RN license. Barbara holds a Master of Social Work from Rutgers University and a Bachelor  of Arts in Social Welfare from The Pennsylvania State University.


Lacey McCray,
Intake and Scheduling Coordinator

Lacey McCray

Lacey has been with Springpoint Senior Living since March 2017. Some of her daily tasks include gathering new referrals, processing preliminary payroll, and scheduling home health aides. Her favorite part about her job is the ability to interact with the patients on a regular basis and establish working friendships with the aides. Lacey is a graduate of Manchester Township High School in Ocean County, NJ, and hopes to return to school in the future.


Shann McGwier, RN,
Home Care Staff Nurse

Shann McGuire

Shann, a registered nurse, has been with Springpoint Senior Living since 1991 and her concentration of care has been with the geriatric community. She worked in the long-term care setting for many years and is now in the home-care setting. As a staff nurse, Shann is responsible for meeting with residents and their families and assisting them with the admissions and assessment process. She continues to provide support to both caregivers and residents throughout their care with Springpoint at Home. Shann has been in the nursing field for 40 years and has had experience in family practice, OB/GYN, and with an ENT practice. Shann holds a Bachelor of Science in Foods, Nutrition and Wellness Studies from the University of Alabama and an Associate of Science from Mercer County Community College.


Annette Murphy, MSW, LSW, C-ASWCM ,
Director of Home Care

Annette Murphy

Annette has over 20 years of experience in a variety of healthcare settings. Prior to Annette’s current role as director of home care, she worked as a geriatric care manager and supervisor for Springpoint at Home. She has worked as a director of social work in a long-term care setting—in acute rehabilitation and brain injury rehabilitation, as a support group leader for stroke patients and caregivers, in the activities department with dementia clients, and as an adjunct professor at The College of New Jersey. She is a licensed clinical social worker, a certified advanced case manager, and a member of the National Association of Social Workers. Annette serves on the Board of Directors for the Aging Life Care® Association of New Jersey, is a member of the Hamilton Rotary Club, and is an avid community volunteer. She is a member of the Springpoint Senior Living Ethics Committee and attends the Mercer County Workforce Development Committee meetings. Annette holds a Master of Social Work from Rutgers University and a Bachelor of Arts in Family Studies and Gerontology from Messiah College.


Deborah Gluck Perry, M.Ed.,
Geriatric Care Manager

Deborah Gluck Perry

Deborah has over 25 years of experience in the healthcare industry, serving the community of Tucson, Arizona. Her experience encompasses counseling, social work, community relations, and marketing and sales for the senior population. She has enjoyed her work in numerous skilled nursing and assisted living communities as a referral agent for assisted living homes. Deborah returned to New Jersey in 2009, working as assistant director of the Office of Training and Development for the New Jersey Department of Children and Families. She recently returned to her true passion—elder care—and happily began working in home health as a care manager for Springpoint as Home. She is a member of the Aging Life Care® Association and her expertise as a care manager is in holistic healthy lifestyle advocacy, counseling, and facility placement. Deborah holds a Master of Counseling and Guidance and a Bachelor of Arts in Radio and Television Communications, both from the University of Arizona.


Jessica T. Roberts, RN, BSN, CALA,
Nursing Supervisor

Jessica Roberts

Jessica has been in healthcare for over 25 years, beginning as a candy striper before eventually becoming a registered nurse. Her experience is varied, including work in hospitals, medical-surgical nursing, home care, school health, employee health, camp nursing, geriatrics, and dialysis. After spending seven years as the director of assisted living and memory care at Springpoint’s Meadow Lakes, she transferred to Springpoint at Home over four years ago. She loves meeting new clients and their families and coordinating their care. Jessica is licensed in two states and is a certified assisted living administrator, a certified medication aide trainer, and a certified home health aide instructor. Her regulatory survey experiences show compliancy in Department of Health surveys, the Commission on Accreditation of Rehabilitation Facilities, My InnerView, and The Joint Commission areas. She is active in several organizations, including the national and state chapters of the American Assisted Living Nursing Association, the International Women’s Leadership Association, and the Health Care Advisory Board. Jessica holds a Bachelor of Science in Nursing from Herbert H. Lehman College of the City University of New York.


Michele Woodley, MSW,
Administrative Assistant/Human Resources

A Springpoint Senior Living professional for more than 11 years, Michele joined the Springpoint at Home team in 2018. She is responsible for providing administrative support to the director of care, nursing supervisors, registered nurses, and intake coordinators in the overall operations of office activities according to policies and procedures. She maintains confidentiality of all Springpoint at Home-related information and collaborates with Springpoint’s human resources staff regarding employee benefit programs. Her experience also includes corporate compliance training and comprehensive understanding of the National Association of Social Workers (NASW) Code of Ethics. She has been a member of the NASW since 2013. Michele holds a Master of Social Work and a bachelor’s degree in Sociology and Business Administration as well as a certificate in Gerontology.


Andrea VanCooten,
Intake Scheduling Coordinator/CHHA

Andrea Vancooten

Andrea, an intake scheduling coordinator, began her work with Springpoint Senior Living in 2014 as a certified home health aide (CHHA). She has prior experience as a medical office assistant, residential counselor support person, and teacher’s aide. Andrea processes patient documentation and coordinates physician schedules, manages orders, and handles insurance authorizations. She is pleased to have transitioned to her current position and enjoys her job very much.
